Through NIU we are able to offer ISBE approved documentation for up to 5 hours of your attendance at the ILCMP Summer Online Workshop: RELEVANT MUSIC INSTRUCTION DURING THE PANDEMIC.
In order to receive PD, you will need to do the following:
1. DOWNLOAD the ISBE evaluation form found HERE.
2. Complete the form and rename it with your LAST name followed by first initial.
3. Send an e-mail with the form attached to [email protected]. Please put Summer2020 PD in the subject line.
4. In the e-mail please indicate how many hours you attended. (Attendance will be taken at each session—nothing for you to do for that.)
NOTE: Forms must be received by midnight Sunday, July 19th.
You will receive the official ISBE evidence completion form via e-mail the following week.
Participants outside of Illinois can receive a certificate from IL-CMP for their number of professional development hours completed by sending a request to the above e-mail address (#3).
(If you are unable to complete the form digitally, please print, complete, and scan or take a picture of it.)
